      Schedule an appointment with your workers’ compensation doctor and discuss your injury or condition.

    2. Request a Referral to The6
      Ask your physician to submit a referral for the services recommended as part of your treatment plan.

    3. Specify The6 as Your Provider
      Let your doctor, claims adjuster, and case manager know that you would like to receive care at The6.

    4. Workers’ Compensation Authorization
      Your claims adjuster will review the referral and issue authorization for approved services.

    5. Schedule Your Appointment
      Once authorization is received, our team will contact you to schedule.
